ABSTRACT

El monóxido de carbono es un gas altamente tóxico que se origina principalmente por la combustión incompleta de combustibles fósiles. La intoxicación presenta síntomas inespecíficos que solapan otras patologías y por lo tanto es indispensable la confirmación mediante la medición de la carboxihemoglobina en sangre. El laboratorio incorporó la determinación en el informe del estado ácido base a partir de octubre del 2018, debido a que previamente el médico debía solicitarla frente a la sospecha de una intoxicación. El objetivo del trabajo fue evaluar si esta medida implementada por el laboratorio contribuyó a mejorar el diagnóstico de intoxicación por CO, analizar las características de los pacientes con COHb mayor o igual a 5% y definir un valor de reporte inmediato para la COHb. El 46% de los casos con COHb mayor o igual a 5% no se relacionaban con una intoxicación y/o exposición a CO. De los casos de intoxicación se encontró que el 77% fueron diagnosticados a partir de la sospecha médica y un 23% por hallazgo del laboratorio. Se concluyó que es de mucha utilidad el rol del laboratorio en detectar aquellos casos que no fueron evidentes clínicamente. Existen ciertas patologías como las oncológicas o la enfermedad de Wilson donde se vieron valores elevados de COHb sin presentar intoxicación y se definió finalmente, como valor de reporte inmediato 7% para la COHb. Carbon monoxide is a highly toxic gas that originates mainly from incomplete combustion of fossil fuels. Intoxication causes nonspecific symptoms that overlap with other conditions and, therefore, confirmation by measuring blood carboxyhemoglobin is essential. The laboratory incorporated the measurement in the acid-base status report as of October 2018, as it was previously required to be requested by the physician in case of suspected intoxication. The aim of this study was to evaluate whether this measure implemented by the laboratory contributed to the improvement of the diagnosis of CO intoxication, to analyze the characteristics of patients with COHb greater than or equal to 5% and to define an immediate reporting value for COHb. Overall, 46% of the cases with COHb greater than or equal to 5% were not related to CO poisoning and/or exposure. Of the cases of intoxication, 77% were diagnosed based on medical suspicion and 23% on laboratory findings. It was concluded that the laboratory has a useful role in detecting cases that were not clinically evident. There are certain diseases including different types of cancer or Wilson's disease where elevated COHb values were seen without intoxication and finally, 7% for COHb was defined as the immediate reporting value (AU)

ABSTRACT

The evaluated ammonia volatilization rates and its association with humidity, temperature, and pH in broiler litter in conventional and dark house rearing systems. Evaluations were performed at 0.01, 1.00, and 1.50 m high, using a completely randomized design, at a weekly frequency for 12 weeks­from the sanitary void to the end of the 42-day cycle. Litter temperature had no significant difference for different types of house. Humidity and pH levels varied according to houses and evaluation periods. No ammonia volatilization was detected during initial periods. However, up from the 21st day of rearing, rates started to increase, showing higher concentrations at 0.01 m height. At 1.00 and 1.50 m, values around 20 mL·m­3, were obtained and considered an acceptable exposure level for the broilers and employees directly working in this sector. Yet, broiler breeders should target lower levels due to environmental impacts.

Introdução. O material particulado (MP) emitido pela queima de combustíveis fósseis de origem veicular é a principal fonte de exposição ambiental à elementos potencialmente tóxicos (EPTs) presentes no ar atmosférico dos centros urbanos, entre eles arsênio, níquel e chumbo. Objetivos. Este estudo avaliou os riscos à saúde humana de efeitos não carcinogênicos e efeitos carcinogênicos associados a exposição inalatória de arsênio, níquel e chumbo no MP10 de origem veicular, coletado em uma das estações de monitoramento da qualidade do ar, na cidade de São Paulo, ao longo dos anos 2002, 2006, 2009 e 2012. Métodos. Os dados de concentração dos EPTs foram obtidos pela Companhia Ambiental do Estado de São Paulo (CETESB) e estão disponíveis no website da agência. As amostras semanais foram coletadas em todas as estações do ano, durante 2002, 2006, 2009 e 2012 na estação de monitoramento Cerqueira César, localizada próximo a uma via de intenso tráfego veicular. A análise estatística foi realizada utilizando o Software R (versão 4.0.5) e RStudio (versão 1.2). O software ProUCL foi usado para estimar os valores UCL95%. A avaliação de risco à saúde humana foi realizada de acordo com os métodos da United States Environmental Protection Agency (USEPA). Resultados. As concentrações de arsênio e chumbo no ar apresentaram médias mais elevadas durante o inverno do que nas outras estações (p <0,05). O chumbo apresentou o maior risco de efeitos não carcinogênicos (HQ> 1,0). Arsênio e níquel apresentaram o maior risco de efeitos carcinogênicos, inclusive acima de 1E-06. O risco para ambos os efeitos foi maior no inverno. Conclusão. Esses achados destacam a importância da poluição do ar como fator de risco para a saúde da população, principalmente em centros urbanos com intenso tráfego veicular. Ações para reduzir a exposição à poluição do ar ambiente devem ser priorizadas nas agendas de políticas ambientais e de saúde.


Background. Particulate matter (PM) emitted from the burning of fuels by vehicles is the main source of environmental exposure to potentially toxic elements (PTEs) present in atmospheric air in urban centers, between them arsenic, nickel and lead. Objective. This study evaluated the risk to human health from non-carcinogenic effects and carcinogenic effects associated with airborne arsenic, nickel and lead in PM10 exposure of the vehicular source collected at one of the air quality monitoring stations in the city of Sao Paulo over the years 2002, 2006, 2009 and 2012. Methods. The concentration data of the PTEs were obtained by the Sao Paulo State Environmental Protection Agency (CETESB) and are available on the agency's website. The statistical analysis was performed using the Software R (version 4.0.5) and RStudio (version 1.2). ProUCL software was used to estimate UCL95% values. The risk assessment for human health was carried out according to the methods of the United States Environmental Protection Agency (USEPA). Results. Airborne arsenic and lead concentrations showed higher means during the winter than in other seasons (p<0.05). Lead had the greatest risk of non-carcinogenic effects (HQ>1.0). Arsenic and nickel had the highest risk of carcinogenic effects, including above 1E-06. The risk for both effects was greatest in winter. Conclusion. These findings highlight the importance of air pollution as a risk factor for population health, especially in urban centers with high vehicular traffic. Actions to reduce air pollution exposure should be prioritized in environmental and health policies agendas.

ABSTRACT

SUMMARY: Exposure to air pollution and its pollutants has been associated with important effects on human health since the first years of life, thus it has been seen that exposure to tobacco smoke and wood smoke is directly related to cardiovascular and pulmonary diseases, respiratory and cancers. However, exposure to air pollution during fetal development and its effects on brain structure and function during early childhood and adolescence have been little studied. In this review we have analyzed the literature on prenatal exposure to tobacco and wood smoke and its relationship with hypothalamic development and cognition in the first years of life.The molecular, morphological and physiological aspects of the relationship between pre- and postnatal exposure to tobacco and wood smoke with neural developmental, cognitive and behavioral problems during early childhood and adolescence have not yet been fully clarified. The information available in the scientific literature based on antecedents obtained from epidemiological studies has been negatively affected by confounding variables and great methodological challenges that make it impossible to affirm an exact causal relationship with certainty.


RESUMEN: La exposición a la contaminación del aire se ha asociado con importantes efectos en la salud humana desde los primeros años de vida. Estudios han demostrado con certeza que la exposición al humo de tabaco y humo de leña está directamente relacionada con enfermedades cardiovasculares, pulmonares, respiratorias y cánceres. Sin embargo, la exposición a la contaminación del aire durante el desarrollo fetal y sus efectos a posteriori sobre la estructura y función del cerebro durante la primera infancia y la adolescencia son aún desconocidos. En esta revisión analizamos la literatura sobre la exposición prenatal al tabaco y al humo de leña y su relación con el desarrollo hipotalámico y la cognición en los primeros años de vida. Los aspectos moleculares, morfológicos y fisiológicos de la asociación entre la exposición pre y postnatal al humo de tabaco o al humo de leña con problemas del desarrollo neurológico normal, cognitivos y de comportamiento durante la primera infancia y la adolescencia aún no se han aclarado completamente. La información disponible en la literatura científica basada en antecedentes obtenidos de estudios epidemiológicos ha sido afectada negativamente por variables de confusión y grandes desafíos metodológicos que hacen imposible afirmar una relación directa y causal exacta con certeza.

ABSTRACT

ABSTRACT: During the current coronavirus epidemic, personal protective equipment has become a vital issue. Depending on the procedure, the generation of aerosols increases the risk of infection for healthcare professionals. However, in dentistry, there is a high risk of contamination from blood or tissue fluids, generated in surgical procedures with ultrason ic and high-speed cutting devices. Thus, the creation of a prototype with a functional respiratory protection system offers filtered air at the entrance and exit of the circuit, also benefiting the patient. This experimental and unprecedented air purification system has 99.9 % high efficiency filters, positive and negative pressure control and also generates an internal balance of air flow; its purpose is to maintain the facial seal of the snorkel mask. The results showed that this air purificat ion system could be an option in environments contaminated by aerosol caused by rotary intruments in dentistry.


RESUMEN: Durante la actual epidemia de coronavirus, el equipo de protección personal se ha convertido en un problema vital. Dependiendo del procedimiento, la generación de aerosoles aumenta el riesgo de infección para los profesionales de la salud. Sin embargo, en odontología, existe un alto riesgo de contaminación por sangre o fluidos tisulares, generados en procedimientos quirúrgicos con dispositivos de corte ultrasónicos y de alta velocidad. Por lo tanto, la creación de un prototipo con un sistema funcional de protección respiratoria ofrece aire filtrado a la entrada y salida del circuito, lo que también beneficia al paciente. Este sistema de purificación de aire experimental y sin precedentes tiene 99.9 % de filtros de alta eficiencia, control de presión positiva y negativa y también genera un equilibrio interno del flujo de aire; Su propósito es mantener el sello facial de la máscara de snorkel. Los resultados mostraron que este sistema de purificación de aire podría ser una opción en ambientes contaminados por aerosoles causados por instrumentos rotativos en odontología.

INTRODUCCIÓN: Las placas pleurales por fibras de asbesto se caracterizan por lesiones compuestas por tejido fibroso que se ubican en la pleura parietal. Suelen aparecer hasta en un 3% a 58% de los trabajadores que estuvieron expuestos a fibras de asbesto, y en un 0,5% a 8% en población general. El OBJETIVO de este artículo es presentar dos casos clínicos de pacientes a los que se les detectó en la radiografía de tórax alteraciones pleurales posiblemente asociadas a la exposición a fibras de asbesto. CASOS CLÍNICOS: Caso 1. Paciente de sexo masculino de 49 años, trabajador de la construcción con antecedentes de exposición a fibras de asbesto. Se le realizó una radiografía de tórax con técnica OIT (Organización Internacional del Trabajo), la que evidenció alteraciones pleurales focales. Una tomografía axial computarizada de tórax, confirmó la presencia de placas pleurales. Caso 2. Paciente de sexo femenino de 79 años, jefa de hogar, sin exposición laboral conocida a fibras de asbesto. En una radiografía de tórax anteroposterior, se observó la presencia de una placa pleural focal. Se solicitó una tomografía axial computarizada de tórax, la que confirmó la presencia de placas pleurales. CONCLUSIÓN: La radiografía de tórax con técnica OIT es el instrumento básico para la identificación de enfermedades relacionadas con la exposición a fibra de asbesto. El estudio debe ser completado con una tomografía axial computarizada de tórax cuya sensibilidad es mayor permitiendo detectar tempranamente las anomalías pleurales. Es fundamental la historia ocupacional detallada ya que constituye el método más fiable y práctico para medir la exposición a fibra de asbesto.


INTRODUCTION: Asbestos fiber pleural plaque is characterized by lesions composed of fibrous tissue that are located in the parietal pleura. They usually appear in up to 3 to 58% of workers who were exposed to asbestos fiber, and 0.5 to 8% in the general population. The OBJECTIVE of this article is to present two clinical cases of patients who were detected in the chest radiograph pleural alterations associated with exposure to asbestos fibers. CLINICAL CASES: First case: 49-year-old male patient, building worker with a history of exposure to asbestos fibers. Focal pleural alterations were detected by a chest x-ray performed according ILO (International Labour Organization) technique. The presence of pleural plaques was confirmed in a computed tomography of the chest. Second case: Holder, without occupational exposure to asbestos fibers. An anteroposterior chest radiography showed the presence of focal pleural plaque in a CT scan of the chest. CONCLUSION: Chest x-ray with ILO technique is the basic instrument for the identification of diseases related to asbestos fiber exposure. The study should be completed with a CT scan of the chest whose sensitivity is greater, allowing early detection of pleural abnormalities. Detailed occupational history is essential, as it is the most reliable and practical method to measure asbestos fiber exposure.

ABSTRACT Objective To estimate the frequency of secondhand smoke exposure among patients with asthma. Methods A cross-sectional study of asthma patients and non-asthmatic controls using questionnaires to identify secondhand smoke exposure at home, school, work, and public places. Results We studied 544 severe asthma patients, 452 mild/moderate asthma patients, and 454 non-asthmatic patients. Among severe patients, the mean age was 51.9 years, 444 (81.6%) were female, 74 (13.6%) were living with a smoker, 383 (71.9%) reported exposure in public spaces and, of the 242 (44.5%) who worked/ studied, 46 (19.1%) reported occupational exposure. Among those with mild/moderate asthma, the mean age was 36.8 years, 351 (77.7%) were female, 50 (11.1%) reported living with a smoker, 381 (84.9%) reported exposure in public settings and, of the 330 (73.0%) who worked/ studied, 58 (17.7%) reported occupational exposure. An association between secondhand smoke exposure and disease control was found among patients with mild/moderate asthma. Among those interviewed, 71% of severe asthma patients and 63% of mild/moderate asthma patients avoided certain places due to fear of secondhand smoke exposure. Conclusion Secondhand smoke exposure is a situation frequently reported by a significant proportion of asthma patients. Individuals with asthma are exposed to this agent, which can hamper disease control, exacerbate symptoms and pose unacceptable limitations to their right to come and go in public settings.


RESUMO Objetivo Estimar a frequência de exposição à fumaça secundária do cigarro entre pacientes com asma. Métodos Estudo transversal, que avaliou pacientes com asma e controles sem asma, por meio de questionários, para identificar a exposição secundária à fumaça do cigarro no ambiente domiciliar, escolar, no trabalho e em ambientes públicos. Resultados Estudamos 544 asmáticos graves, 452 com asma leve/moderada e 454 sem asma. Entre os asmáticos graves, a média de idade foi de 51,9 anos, 444 (81,6%) eram do sexo feminino, 74 (13,6%) tinham fumantes em sua residência, 383 (71,9%) relataram exposição em ambientes públicos e, dos 242 (44,5%) que trabalhavam e/ou estudavam, 46 (19,1%) admitiram exposição ocupacional. Entre asmáticos leves/moderados, a média de idade foi de 36,8 anos, 351 (77,7%) eram do sexo feminino, 50 (11,1%) afirmaram haver tabagistas em sua residência, 381(84,9%) relataram exposição em ambientes públicos e, dos 330 (73,0%) que trabalhavam e/ou estudavam, 58 (17,7%) referiram exposição ocupacional. Encontrou-se associação entre exposição à fumaça secundária do cigarro e controle da doença entre pacientes com asma leve/moderada. Entre os entrevistados, 71% dos pacientes asmáticos graves e 63% daqueles com asma leve/moderada relataram evitar frequentar certos ambientes pelo receio da exposição à fumaça secundária do cigarro − relato mais associado aos pacientes com asma grave. Conclusão A exposição secundária à fumaça do cigarro é uma situação frequente e relatada por uma proporção significativa de asmáticos. Indivíduos com asma encontram-se expostos a este agente, que pode dificultar o controle da doença, exacerbar sintomas e lhes impor limitação inaceitável ao direito de ir e vir em ambientes públicos.

BACKGROUND@#We previously demonstrated that continuous exposure to nitrous acid gas (HONO) for 4 weeks, at a concentration of 3.6 parts per million (ppm), induced pulmonary emphysema-like alterations in guinea pigs. In addition, we found that HONO affected asthma symptoms, based on the measurement of respiratory function in rats exposed to 5.8 ppm HONO. This study aimed to investigate the dose-response effects of HONO exposure on the histopathological alterations in the respiratory tract of guinea pigs to determine the lowest observed adverse effect level (LOAEL) of HONO.@*METHODS@#We continuously exposed male Hartley guinea pigs (n = 5) to four different concentrations of HONO (0.0, 0.1, 0.4, and 1.7 ppm) for 4 weeks (24 h/day). We performed histopathological analysis by observing lung tissue samples. We examined samples from three guinea pigs in each group under a light microscope and measured the alveolar mean linear intercept (Lm) and the thickness of the bronchial smooth muscle layer. We further examined samples from two guinea pigs in each group under a scanning electron microscope (SEM) and a transmission electron microscope (TEM).@*RESULTS@#We observed the following dose-dependent changes: pulmonary emphysema-like alterations in the centriacinar regions of alveolar ducts, significant increase in Lm in the 1.7 ppm HONO-exposure group, tendency for hyperplasia and pseudostratification of bronchial epithelial cells, and extension of the bronchial epithelial cells and smooth muscle cells in the alveolar duct regions.@*CONCLUSIONS@#These histopathological findings suggest that the LOAEL of HONO is < 0.1 ppm.

Resumo Objetivo Analisar os sinais e sintomas apresentados por médicos residentes das clínicas cirúrgicas e anestesiologia expostos à fumaça cirúrgica. Métodos Estudo de coorte prospectivo realizado com médicos residentes expostos a fumaça cirúrgica em um hospital universitário. Houve um acompanhamento durante 17 meses dos residentes ingressantes nos anos de 2015 e 2016, que atendiam aos critérios de estar regularmente matriculado na residência de clínica cirúrgica ou anestesiologia e não ser tabagista. O instrumento de coleta de dados foi composto de dados sociodemográficos e acadêmicos e dos sinais e sintomas relacionados com a inalação da fumaça cirúrgica, citados na literatura. A análise de dados ocorreu de forma descritiva e inferencial, por testes estatísticos e medidas de efeito. Resultados A amostra foi composta por 39 residentes, cuja maioria era do sexo masculino (56,4%) e idade abaixo dos 30 anos (74,3%). Prevaleceram residentes da ginecologia e obstetrícia (30,8%), seguidos de cirurgia geral (28,2%) e anestesiologia (20,5%). Ardência na faringe (p=0,030), náusea e vômito (p=0,018) e irritação dos olhos (p=0,050) incidiram ainda no primeiro ano de residência. O risco de desenvolver ardência de faringe foi 7,765 vezes (p=0,019) no sexo feminino em relação ao masculino. Conclusão Os sinais e sintomas analisados incidiram em até 12 meses do início da residência e o risco de apresentar ardência de faringe foi maior no sexo feminino, o que indica a exposição aos riscos da inalação da fumaça cirúrgica e, portanto, a necessidade de adoção de medidas de proteção individuais e coletivas.


Resumen Objetivo analizar las señales y síntomas presentados por médicos residentes de clínica quirúrgica y anestesiología expuestos al humo quirúrgico. Métodos estudio de cohorte prospectivo realizado con médicos residentes expuestos al humo quirúrgico en un hospital universitario. Hubo un seguimiento durante 17 meses de los residentes que ingresaron en 2015 y 2016, que cumplían los criterios de estar regularmente matriculados en la residencia de clínica quirúrgica o anestesiología y no ser fumadores. El instrumento de recolección de datos fue compuesto por datos sociodemográficos y académicos y por señales y síntomas relacionados con la inhalación de humo quirúrgico, citados en la literatura. El análisis de datos se realizó de forma descriptiva e inferencial, por pruebas estadística y medidas de efecto. Resultados la muestra fue compuesta por 39 residentes, cuya mayoría era de sexo masculino (56,4%) y menores de 30 años (74,3%). Prevalecieron residentes de ginecología y obstetricia (30,8%), seguidos de cirugía general (28,2%) y anestesiología (20,5%). Ardor de faringe (p=0,030), náuseas y vómitos (p=0,018) e irritación de ojos (p=0,050) incidieron en el primer año de residencia. El riesgo de desarrollar ardor de faringe fue 7,765 veces (p=0,019) en el sexo femenino con relación al masculino. Conclusión las señales y síntomas analizados incidieron hasta 12 meses desde el inicio de la residencia y el riesgo de presentar ardor de faringe fue mayor en el sexo femenino, lo que indica una exposición a los riesgos de inhalación de humo quirúrgico y, por lo tanto, la necesidad de adoptar medidas de protección individuales y colectivas.


Abstract Objective To analyze the signs and symptoms presented by doctors in surgery and anesthesiology residency programs exposed to surgical smoke. Method Prospective cohort study with resident doctors exposed to surgical smoke in a teaching hospital. There was 17-month follow-up of residents from the years 2015 and 2016, who met the criteria of being regularly enrolled in a surgery or anesthesiology residency and not being a smoker. The data collection instrument was composed of sociodemographic, academic data and the signs and symptoms related to the inhalation of surgical smoke, cited in the literature. The data analysis was descriptive and inferential, by statistical tests and measures of effect. Results The sample consisted of 39 residents, of which most were male (56.4%) and below 30 years old (74.3%). There was a prevalence of gynecology and obstetrics residents (30.8%), followed by general surgery (28.2%) and anesthesiology (20.5%). Burning in the pharynx (p=0.030), nausea and vomiting (p=0.018) and eye irritation (p=0.050) occurred in the first year of residence. The risk of developing burning in the pharynx was 7.765 times greater (p=0.019) in females when compared to males. Conclusion The signs and symptoms analyzed occurred within 12 months of the beginning of the course and the risk of burning in the pharynx was higher in females, which indicates exposure to the risks of inhalation of surgical smoke and, therefore, points to the need for the adoption of individual and collective protection measures.

ABSTRACT Objective: Identify the association between exposure to fine particulate matter and hospitalizations due to respiratory diseases in children up to ten years of age in the city of Cuiabá, Mato Grosso, stratifying the analysis by sex and calculating excess costs. Methods: Ecological study of time series. The dependent variable was daily hospitalizations according to the 10th Revision of the International Classification of Diseases (ICD10): J04.0, J12.0 to J18.9, J20.0 to J21.9 and J45.0 to J45.0. The independent variables were the concentration of fine particulate, estimated by a mathematical model, temperature and relative air humidity, controlled by short and long-term trends. Generalized additive model of Poisson regression was used. Relative risks, proportional attributable risk (PAR) and excess hospitalizations and their respective costs by the population attributable fraction (PAF) were calculated. Results: 1,165 children were hospitalized, 640 males and 525 females. The mean concentration, estimated by the mathematical model, was 15.1±2.9 mcg/m3 for PM2.5. For boys, there was no significant association; for girls a relative risk of up to 1.04 of daily hospitalizations due to respiratory diseases was observed for exposure to PM 2.5 in lags 1, 2 and 6. Increase of 5 µg/m3 in these concentrations increased the percentage of the risk in 18%; with an excess 95 hospital admissions and with excess expenses in the order of US$ 35 thousand. Conclusions: Significant effect in daily hospitalizations due to respiratory diseases related to exposure to fine particulate matter was noted for girls, suggesting the need for stratification by sex in further studies.


RESUMO Objetivo: Identificar associação entre exposição ao material particulado fino e internações por doenças respiratórias em crianças de até dez anos de idade, no município de Cuiabá, MT, estratificando a análise por sexo e calculando excesso de custos. Métodos: Estudo ecológico de séries temporais, sendo a variável dependente o número diário de internações por doenças respiratórias, segundo a 10a Revisão da Classificação Internacional de Doenças (CID): J04.0, J12.0 a J18.9, J20.0 a J21.9 e J45.0 a J45.0. As variáveis independentes foram a concentração do particulado fino, estimada pelo modelo matemático, além de temperatura e umidade relativa do ar, controladas pelas tendências de curta e longa duração. Foram calculados riscos relativos, risco atribuível proporcional (RAP) e excessos de internações e seus respectivos custos pela fração atribuível populacional (FAP). Resultados: Foram internadas 1.165 crianças (640 meninos e 525 meninas). A concentração média estimada do particulado fino foi 15,1±2,9 mcg/m3 para particulado fino (PM2,5). Para meninos, não houve associação significativa. Para meninas, observou-se risco relativo (RR) de até 1,04 para o número diário de internações por doenças respiratórias em associação à exposição ao PM2,5 nos lags 1, 2 e 6. O aumento de 5 mcg/m3 nessas concentrações associou-se ao aumento do risco em 18% e o RAP atribuído à exposição foi de 20% das internações das meninas, com excesso de 95 internações e de gastos da ordem de R$ 105 mil. Conclusões: Houve associação da exposição ao particulado fino e número de internações por doenças respiratórias de meninas, sugerindo a necessidade de estratificação por sexos em estudos posteriores.

ABSTRACT

Los incendios forestales representan un problema creciente de la salud pública a nivel mundial, especialmente para la población más vulnerable (niños, ancianos, embarazadas y portadores de enfermedades cardiovasculares o respiratorias crónicas) expuesta al humo y a otros contaminantes aéreos. A diferencia de la contaminación atmosférica habitual de grandes urbes, aquella derivada de los incendios forestales tiene una composición diferente y su ocurrencia es esporádica y difícil de prever. La exposición a contaminantes atmosféricos derivados de incendios forestales se asocia a aumento de la morbilidad respiratoria y cardiovascular, mediada por una respuesta inflamatoria pulmonar y sistémica, estrés oxidativo y disfunción endotelial. En sujetos expuestos a humo de incendios forestales se ha observado un aumento en la producción de citoquinas pro-inflamatorias, activación endotelial y disfunción del sistema nervioso autónomo, que produce daño tisular, aumento de los mecanismos protrombóticos, aumento de la presión arterial y cambios en el ritmo cardiaco. Esta revisión analiza los mecanismos que han sido involucrados en generar efectos nocivos para la salud de seres humanos expuestos a material particulado y gases emanados de incendios forestales.


Wildfires represent a growing global public health issue, especially to the most vulnerable segment of the population (children, old people, pregnant women, patients with cardiovascular or respiratory diseases) exposed to smoke and other air borne contaminants generated from these events. In contrast to great cities ' usual atmospheric pollution, that derives from forest fires differ in composition and its occurrence is sporadic and usually unpredictable. Exposure to atmospheric pollutants derived from forest fires has been associated to increased respiratory and cardiovascular morbidity, mediated by an inflammatory systemic response, oxidative stress and endothelial dysfunction. In people exposed to forest fire smoke an increased production of pro-inflammatory cytokines, endothelial activation and autonomic nervous system dysfunction has been observed, that leads to tissue injury, increased prothrombotic response, increased blood pressure and changes in heart rhythm. This review analyzes the mechanisms that have been involved in generating harmful health effects in humans exposed to inhaled particulate matter and gases steaming from wildfires.

ABSTRACT BACKGROUND: Exposure to some air pollutants is associated with cardiovascular diseases. The objective of this study was to quantify the effect of exposure to fine particulate matter in hospitalizations due to ischemic heart disease and the costs to the healthcare system. DESIGN AND SETTING: Time-series ecological study conducted in Taubaté, Brazil. METHODS: Data on hospitalizations due to ischemic heart diseases (ICD I-20 to I-24) in the municipality of Taubaté (SP), Brazil, among adults of both sexes aged 40 years and over, from August 2011 to July 2012, were obtained from DATASUS. Fine particulate matter (PM2.5) concentrations were estimated from a mathematical model. Poisson regression was used in statistical analyses to estimate the relative risks of exposure to PM2.5 for both sexes and after stratification according to sex. The excess of hospitalizations and consequent excess expenditure for the healthcare system were calculated. RESULTS: There were 1040 admissions, among which 382 had ischemic heart diseases (257 males). Themean PM2.5 concentration was 13.2 µg/m3 (SD = 5.6). Significant effects from exposure were noted 4and 5 days after exposure (lag 4 and lag 5) for both sexes and for male sex; for female sex, the effect was 2 days after exposure (lag 2). There were 59 excess hospitalizations for an increase in PM2.5 concentration of 5 µg/m3 and excess expenditure of US$ 150,000 for the National Health System. CONCLUSIONS: An excess of hospital admissions due to ischemic heart disease, with excess expenditure, was identified consequent to PM2.5 exposure.

ABSTRACT Objective: To compare the prevalence of signs and symptoms related to inhalation of surgical smoke among surgical technologists and non-surgical technologists nursing professionals. Method: A cross-sectional study with 46 professionals from a university hospital. To evaluate the prevalence, an instrument with signs/symptoms related to the inhalation of electrocautery smoke was used. To verify the comparison between the prevalence of signs/symptoms, the Fisher's exact test was performed. Results: Higher prevalence of all signs/symptoms among surgical technologists, with a statistically significant difference between the act of instrumenting with the presence of at least one signs/symptoms related to inhalation of surgical smoke (p=0.01); eye irritation (p=0.02); irritation of nasal mucosa and oral cavity (p=0.03); headache (p=0.04). Conclusion: The presence of problems related to surgical smoke in nursing workers elicits more attention. Implications for practice: Health units must be aware of the risk of such exposure and take measures to preventing it.


RESUMEN Objetivo: Comparar la prevalencia de signos y síntomas relacionados a la inhalación del humo quirúrgico entre trabajadores de enfermería instrumentadores y no-instrumentadores. Método: Estudio transversal realizado con 46 trabajadores en un hospital universitario. Para evaluación de la prevalencia, se utilizó un instrumento con signos/síntomas relacionados con la inhalación del humo del electrocauterio. Para la comparación entre prevalencia de signos/síntomas, fue realizada la prueba exacta de Fisher. Resultados: Mayor prevalencia de todos los signos/síntomas entre instrumentadores, con diferencia estadísticamente significante entre el acto de instrumentar con la presencia de por lo menos un signo/síntomas relacionado con la inhalación del humo quirúrgico (p=0,01); irritación en los ojos (p = 0,02); de mucosa nasal y cavidad oral (p=0,03); cefalea (p=0,04). Conclusión: La presencia de problemas relacionados al humo quirúrgico en instrumentadores suscita mayor cuidado. Implicaciones para la práctica: Unidades de salud deben considerar los riesgos de esta exposición y adoptar medidas de prevención.

Abstract Purpose: To evaluate the hyaluronic acid (HA) inflammatory reaction, fibroblasts, fibrosis and duration of effect in the dorsal region of tobacco-exposed rats. Methods: Ten Wistar rats were divided into two groups: tobacco-exposed-group (TEG;n=5) and air-control-group (CG;n=5). The TEG animals were tobacco-exposed twice a day, 30-minutes/session, during 60 days. After this period, all animals received 0.1 mL HA subcutaneous injection in the dorsal area. The volume of HA was measured immediately after HA injection and weekly using a hand-caliper in nine weeks. After this period, all the animals were euthanized, and a specimen of was collected to evaluate inflammatory cells, fibroblasts, and fibrosis by HE. Results: This study showed a higher inflammatory reaction in TEG than CG: inflammatory cell-count (CG: 1.07±0.9; TEG: 8.61±0.36, p<0.001); fibroblast count (CG: 2.92±0.17; TEG: 19.14±0.62, p<0.001), and fibrosis quantification (CG: 2.0; TEG: 3.75, p<0.001). The analysis of the HA volume in nine weeks in the dorsal region did not show a difference between groups (p=0.39). Conclusions: This study suggested that the HA injection in the TEG caused an increase in inflammatory cell count, fibroblast, and fibrosis quantification when compared to the CG. There was no difference in the duration of effect of HA between the groups.

BACKGROUND@#Although lead neurotoxicity is a known phenomenon, it can often be missed at a primary or secondary care level especially if detailed environmental exposure history is missed.@*METHODS@#This is an outbreak investigation where we observed 15 pediatric cases with neurologic signs and symptoms clustered in a slum area known for an unorganized artificial jewelry industry. Their clinical, biochemical, and epidemiological features were compared with 14 other children from the same region reporting with non-neurological symptoms who were considered as unmatched controls.@*RESULTS@#Cases with neurological manifestations had a higher in-house lead smelting activity [OR 7.2 (95% CI 1.4-38.3)] as compared to controls. Toddlers below 3 years of age were more vulnerable to the effects of lead.@*CONCLUSION@#This study emphasizes that many focal sources of lead poisoning still remain especially in the unorganized sector. In cases presenting with unexplained neurotoxicity, specific occupational and environmental inquiry for chemical poisoning, with special consideration for lead, should be actively pursued.

The inhalation of naphthalene used as deodorant balls in public toilets could be an important cancer risk factor. The atmospheric concentration of naphthalene in public toilets (C(in)) was estimated both by a polyurethane foam passive air sampler (PUF-PAS) deployed in nine public toilets in Seoul, Korea and by a steady-state indoor air quality model, including emission estimation using Monte-Carlo simulation. Based on the estimated C(in), cancer risk was also assessed for cleaning workers and the general population. The steady-state C(in) estimated using the estimated emission rate, which assumed that air exchange was the only process by which naphthalene was removed, was much greater than the C(in) value measured using PUF-PAS in nine public toilets, implying the importance of other removal processes, such as sorption to walls and the garments of visitors, as well as decreased emission rate owing to wetting of the naphthalene ball surface. The 95 percentile values of cancer risk for workers based on the estimation by PUF-PAS was 1.6×10⁻⁶, whereas those for the general public were lower than 1×10⁻⁶. The results suggested that naphthalene deodorant balls in public toilets may be an important cancer risk factor especially for the cleaning workers.

BACKGROUND: Metalworking fluids (MWFs) are mixtures with inhalation exposures as mists, dusts, and vapors, and dermal exposure in the dispersed and bulk liquid phase. A quantitative risk assessment was performed for exposure to MWF and respiratory disease.METHODS: Risks associated with MWF were derived from published studies and NIOSH Health Hazard Evaluations, and lifetime risks were calculated. The outcomes analyzed included adult onset asthma, hypersensitivity pneumonitis, pulmonary function impairment, and reported symptoms. Incidence rates were compiled or estimated, and annual proportional loss of respiratory capacity was derived from cross-sectional assessments.RESULTS: A strong healthy worker survivor effect was present. New-onset asthma and hypersensitivity pneumonitis, at 0.1 mg/m3 MWF under continuous outbreak conditions, had a lifetime risk of 45%; if the associated microbiological conditions occur with only 5% prevalence, then the lifetime risk would be about 3%. At 0.1 mg/m3, the estimate of excess lifetime risk of attributable pulmonary impairment was 0.25%, which may have been underestimated by a factor of 5 or more by a strong healthy worker survivor effect. The symptom prevalence associated with respiratory impairment at 0.1 mg/m3 MWF was estimated to be 5% (published studies) and 21% (Health Hazard Evaluations).CONCLUSION: Significant risks of impairment and chronic disease occurred at 0.1 mg/m3 for MWFs in use mostly before 2000. Evolving MWFs contain new ingredients with uncharacterized long-term hazards.

ABSTRACT

Radon is a naturally occurring radioactive material that is formed as the decay product of uranium and thorium, and is estimated to contribute to approximately half of the average annual natural background radiation. When inhaled, it damages the lungs during radioactive decay and affects the human body. Through many epidemiological studies regarding occupational exposure among miners and residential exposure among the general population, radon has been scientifically proven to cause lung cancer, and radon exposure is the second most common cause of lung cancer after cigarette smoking. However, it is unclear whether radon exposure causes diseases other than lung cancer. Media reports have often dealt with radon exposure in relation to health problems, although public attention has been limited to a one-off period. However, recently in Korea, social interest and concern about radon exposure and its health effects have increased greatly due to mass media reports of high concentrations of radon being released from various close-to-life products, such as mattresses and beauty masks. Accordingly, this review article is intended to provide comprehensive scientific information regarding the health effects of radon exposure.

RESUMEN Objetivos Describir e interpretar las metodologías y resultados de los principales estudios relacionados con el monitoreo de contaminación atmosférica en ciclistas. Métodos Se buscaron y analizaron las investigaciones a nivel nacional e internacional de los últimos diez años en las bases de datos Cochrane, Scopus, Embase, Science Direct y Pubmed. La búsqueda se realizó de Agosto a Septiembre del año 2017 usando los siguientes descriptores de búsqueda para MeSH: air pollution, bicycling, environmental exposure, enviromental health, inhalation exposure, environmental pollutants, transportation, public health y toxicology, y para DeCS: contaminación del aire, ciclismo, exposición a riesgos ambientales, contaminantes ambientales, inhalación, transportes, salud pública y toxicología. Resultados Se identificaron diecinueve artículos publicados elegibles. La mayoría de los estudios se realizaron en Europa y Estados Unidos. En Latinoamérica se han reportado cuatro estudios. Otros estudios buscaban comparar la exposición en distintas rutas en bicicleta, comparar trayectos de una misma ruta, y otros determinar la relación distancia vs exposición. De igual manera, variables como el tráfico vehicular, la distancia a las fuentes de emisión y el tipo de ciclorruta, juegan un papel fundamental en la exposición a contaminantes en ciclistas. Conclusiones Se encontraron diversas variables que influyen de forma directa o indirectamente en la exposición a la contaminación del aire de los usuarios de bicicleta, así como algunos factores que pueden reducir la exposición a estos contaminantes.(AU)


ABSTRACT Objectives To describe and interpret the methodologies and results of the main studies related to the monitoring of exposure of cyclists to air pollution. Results Nineteen eligible published articles were identified. Most studies were conducted in Europe and the United States. Four studies have been reported in South America. Other studies seek comparisons on different bicycle routes, compare exposure during variations of the same route, and others determine the distance-exposure relationship. In the same way, variables such as vehicular traffic, distance to the emission sources and the type of bike path, play a fundamental role in the exposure to pollutants in cyclists. Conclusions Several variables were found that influence, directly or indirectly, the cyclists exposure to air pollution, as well as some factors that can reduce this exposure.(AU)
